Course Submission Requirements

In order to assist New York ACEP ITLS course coordinators in meeting the requirements needed to print course cards/certificates, see the checklist below for your reference. We will be unable to assist any agency that does not submit the following materials as dictated by the timeline.

These timelines are important in making it possible to allow affiliate faculty to attend and audit courses throughout the State. Course auditing helps us to address concerns of coordinators, instructors and students to improve future course offerings.

Pre-Course Submissions (to be received no less than 2 weeks prior to course)
The course coordinator must submit notification of planned courses at least 90 days prior to the propsed course.   • Course Schedule(s)
  • Course Outline
  • Instructor Names
Post-Course Submissions
Upon completion of the course, Course Coordinators must submit the following materials within ten (10) days:
  • Faculty/Student Roster
  • Course Registrant Evaluations (copy)
  • All  instructor monitor forms
  • Sign-in Sheets
  • Student registration fees: $20 per student
Once all documentation is received and approved by New York ACEP ITLS, approval will be given for course coordinators to print cards and certificates from the ITLS Course Management System.
 
 Upon completion of steps above, providers will receive
 ITLS card directly from the course coordinator
