Agency Requirements
New York ACEP is excited about the prospect of being able to make ITLS available in other areas. To establish your agency as a ITLS education site, you will need to submit a letter of intent on the following materials:

Agency offering ITLS

Course location

Name and contact information for your course medical director

Medical director's Curriculum Vitae

Letter from director acknowledging acceptance of the responsibilities associated with serving as medical director as well as an understanding of the ITLS curriculum and a willingness to participate in courses (template attached)

Documentation of course faculty certification as ITLS instructors
Estimated number of providers/instructors you foresee training per year

Statement of acceptance of the responsibilities associated with serving as coordinator along with compliance of all New York ITLS policies.
